PMDD Symptom Tracker
Print this Symptom Tracker and use it to keep record of how you are feeling every
day. Rate each symptom as severe, moderate, or mild (or none if you don't have the
symptom). This is done by placing an "X" in 1 of the 4 boxes next to each symptom.
On the line that says "menstruation," place an "X" on the days you are having your
period.
As you complete your Symptom Tracker, draw a line connecting the "X"s. This will
help you and your doctor spot any trends.

with you when you visit your doctor. This tool will help you and your doctor determine
if you have premenstrual dysphoric disorder (PMDD).
